Sanofi Global Health Unit – PharmAccess Partnership: Zanzibar Digital NCD Care Bundles
Remote monitoring for diseases like HIV and tuberculosis is uncommon, but for hypertension and diabetes, scaling this model requires building on current efforts and involving local governments. Zanzibar lacks group-based care for these diseases, but the Ministry of Health is interested in developing this approach, leveraging local communities.

Objectives
The Project is an integrated patient- centered model of care aiming at improving diagnosis and disease management for patients with cardio-metabolic diseases through a care bundle consisting of access to patient groups meetings, digital self-management support, remote care and medications.
Currently the use of remote monitoring and organization in patient groups is relatively uncommon for other diseases such as HIV and TB, but not non-existent for hypertension and diabetes. To successfully scale this model in the longer term, we believe building on existing initiatives and including local government is essential.
On Zanzibar there are currently no group-based initiatives for hypertensionand diabetes care, though there are expert patient educators active in the community via one of the district hospitals. The NCD-unit of the Zanzibar MoH has already expressed interest to develop a patient group-based approach to hypertension and diabetes care.
The social structures on Zanzibar, with a generally tightly knit community, are expected to fit well with the patient group-based approach, for which we can build on experience in Kenya and on mainland Tanzania, where several groups are active.
Results and milestones
Targeted Reach : 600 patients enrolled in a sponsored NCD care program (Diabetes/CV) – treated & followed
The project aims to achieve the following 4 components:
- Introduce remote care monitoring
- Digital diabetes and hypertension care packages available at acceptable cost levels to patients.
- Higher % controlled blood pressure and blood glucose levels among patients using the package
- NCD Care bundle ready for adoption and scaling by partners
